OK...now CNP's can do colonoscopies???
Your input is interesting. My concern is for patients undergoing a colonoscopy by a provider who is qualified to perform the procedure. I have been "assisting" doing cases with GI Physicians/fellows for 5 years and I have not heard of someone with minimal training completing this procedure. That precisely is my gripe. I will not "change with the times" if patient care is compromised. This advice, in my opinion is inappropriate on this particular issue.

I work on the East coast at a hospital and have just found out that a NP is going to train to do colonoscopies by a gastroenterologist. I need to know if RNs can take orders for moderate sedation from a NP? This NP was recently was over heard asking another nurse "what are the reversals again"? This NP was also over heard asking what a pedunculated polyp was. I feel like I know more than him simply because I have been doing procedures for 5 years. I need help convincing everyone in my department that this is not appropriate pt. care. I agree that NP's could possibly do Flex-sigs, but a colonoscopy?? Thanks everyone in advance for input!

Reconstitution of India Ink
Hi All, I have moved to the East Coast and in the Midwest used a product called Spot for marking the lining of the colon. The India ink was prefilled. Now I have to reconstitute it when I need it. It comes in a capsule 0.5 cc. The pharmacist has me using o.9 to prepare for injection. What is everybody else using to mark the colon? Prefilled or doing the mixing your self? Also what are your specific guidelines for preparation of India ink. Thanks!

room turnover time
Hi all! Turnover time ..................so much to do, so little time. I would say that realistic turnover time in a lab with 3 RN's 2 techs, can be 5 minutes if all you are counting is the techs job of removing dirty scope, cleaning cart, replacing scope for next procedure. For RN, transport to recovery, different floor, yes, 8th to 5th usually takes 15 minutes, then upon return, you hook up the next patient to monitors, take assessment, verify npo, allergies, and the doc is in the room wanting to start sedation. Turnover for RNs doing their job properly 35-40 minutes and that might even be pushing it. I'd like to hear feedback/suggestions, can the techs possibly do a little more?

Propofol
:zzzzz We use Demerol and Versed mostly! I was at an ERCP conference in Milwaukee and there does not appear to be a push for RNs to give Propofol. Too many issues and like said deeper sedation than we are trained to manage as RNs. We are not paid enough money to be responsible for deeper sedation!

Endoscopy and Call
Hi all. We have call every 10th weekend...Fri.,sat,sun, then every 9th week day or so. I'm late 9:30 to 6:00 so I often get stuck doing the after office hours cases which is CRAPPY! There is one RN on call weekends and one endo tech. It's supposed to be emergencies only but if its one of the Ladies you can figure on putting in lots of time on the weekend....what is it with the women docs??
