Hi Jessiedog, Thats a great story. It must have added some fun to your day. In my neck-of -woods, we have many people who speak only spanish and I speak SOME spanish as in not completely fluent. My staffing nurse thinks it's funny to send me to a totally spanish speaking household...lol it's either sink or swim, somehow I learned to swim REAL quick. Nancy :)

US Nurses Wishing to Work Overseas
Hi Suzanne, I would like to move to England. I have been doing some research on it. I am wondering if it would be possible to go to school over there to finish up or should I finish here. I eventually will probably move back here. I don't have much left to take in school for my Associates or Bachelors. Mainly just the Nursing program. I currently am an LPN working with hospice and have an extensive Nursing home back ground. I have already began to gather forms. I have a 12 year old daughter who is very enthusiastic about this and has brought all her grades up in school in the hopes that this will work out.

Does anyone else ever get tired of "stupid" nurses?
Lazy, Lazy, Lazy... Start writing them up, oh, thats right, nobody will do anything about it. In My 4 years as a LTC nurse, I learned that Nursing homes are "Wear houses" for the elderly and infirmed and some of them are under the guise of being very ritzy places (ALFs) I am liking more and more the Group home thing, 10 patients max. decent hard working staff with little turn over rate is very low. Usually owned by disgruntle LTC nurses. Patients are treated like family members and well cared for. Things aren't completely perfect all the time, don't get me wrong, but, I have to say that they are much better that the "nursing homes". I am hoping that this trend takes off successfully and runs corporate nursing homes out of business. Nancy:Reindeer:

MORPHINE and Dying Patients
ScooterRN52, That is a great way to think of pain. I always check it first with my vital signs so why not. Cool, Thank you, and I will pass it on at my hospice.

MORPHINE and Dying Patients
To all who have little to no experience with MSO4, Learn all you can and read as much as possible about Morphine and pain. This is a very important drug with dying pts. Many nurses are afraid of it. There is nothing to fear as long as you use it correctly and follow your 5 rights and it can be the difference between a painful, Oxygen deprived, traumatic nightmare for Pt, family, and nurse or a peaceful calm and quite passing. I have been in your shoes about the morphine dilemma, and it is very scary. When I got off my bottom and did research on my own, which still continues to this day, I learned a great deal and was no longer afraid of morphine and haven't ever since then, questioned doctors orders or my actions. There are alternatives for pts with morphine allergies. I urge any nurse who works with dying pts. to educate yourself and know your tools and how they work.

MORPHINE and Dying Patients
I have been a hospice continuous care nurse for 7+ years and I have no reservations about it what so ever. There is nothing worse than seeing an activly dying pt in pain and uncomfortable. I sometimes wonder if we give enough. Some docs are not comfortable (Usually non hospice docs) and won't order it titrated Examp. 0.25 mL - 2mL prn Q 2 hours. That can really tie a hospice cc nurse's hands and then were caught between the family wanted and pleading for me and the case mgr RN to do something when it's all in the docs hands or the family is scared of the morphine. It is also important to remember to get ms order for dyspnea/labored breathing. Does any one really believe that the pt is going to come out of this with drug problem!

hi, this is a little humor in the middle of a family drama I was taking care of a pt's final paper work and getting ready to send my pt with the funeral home transport. I had family member coming in one by one to say their goodbyes, before the pt left the home for the last time and this paticular pt had a beloved pet who was left behind, an adorable dauschhound. between family members the dog got up on the bed and crawled under the sheet and laid down between the pt's legs very quiet and very still. I was busy counting narcotics and doing paper work and was only half aware of what was happening. the pt's eldest daughter came in to say goodbye. well, before I could look up from my paper work and warn the daughter, the pt's daughter was bending over to kiss the pt and the dog moved from under the sheets the daughter jumped up and screamed! when the dog popped out of the sheet, the entire family was in the room, saw what had happened and everyone was laughing hysterically. everyone had felt as though mom was, for one last time, cheering them up.

Let's dialyze the actively dying pt....
Babies and children and always the hardest in a hospice situation. (At least for me it is.)

Let's dialyze the actively dying pt....
I think it is also important to remember that not only the client is ours, but also the family is included as clients as well. They too need us as much as the actual client. If the family still has trouble after we have done and said all we can, then we have the patient-family concelors to fall back on, and/or clergy. NancyS
