Families refusing certain SNF
I run into this situation many times at work. You must tell the patient that you cannot delay discharge on matters of preference and that you fulfilled your obligations to them by providing choices in discharge by giving 3 SNFs. Have a HIN letter handy and let them know that they have the right to appeal their discharge but may be liable for the hospital bill from the time the hospitalist deems them appropriate for going home (or in this case going to SNF). More often than not, the mere thought of having to pay for their hospital stay out of pocket will scare them away from delaying. Also let them know that once discharged to a SNF, they have the ability to transfer if arrangements can be made

New Medicare Guideline's Affect On Case Management
As a case manager, I am both curious and excited as to what the future will hold for us in our specialty. With changes in how the government will reimburse hospitals for things such as readmissions, a greater amount of responsibility will be put on hospitalists and case managers to do their job effectively. Diagnosis' that are notorious for high readmission rates such as heart failure and pneumonia will start being monitored much more closely by Medicare. A sign of things to come is the fact that hospitals are now looking at having a minimum of an RN to become a case manager because of the fact that they want people with strong clinical backgrounds and critical thinking skills making important decisions. I know of a couple hospitals in Northern California who are requesting that its case managers be masters educated or have a Case Management certification. What are your guys' thoughts on how our profession as medical case managers will evolve in the coming years?

New Case Manager Here that needs some encouragement
im not much of a crier but i had my first real shift today (started on wednesday) and i felt like crying. there's so much information to know and i already feel like im not going to succeed. does anyone have any good advice or encouraging words?

RN-MSN education or Case Management Cert any adivice????
having a MSN degree with a concentration i may be bias on the matter but i think its in your best interest to get a MSN if possible. it will give you flexibility in upward movement with the hospital you are with. if you see yourself working for another 20 years then i think your salary will make up for the time and money spent in getting your education as well. cheers

i worked in nurse administration for south SF and was kept in the loop of the Geary SF kaiser as well. expect to see an older bunch of nurses because they have not hired new grad RN's for quite some time now. all med surg units you will see 5 to 1 pt to nurse ratio w/ help from PCAs (nurse assistants). Tele floor 3 to 1 and ICUs 2 to 1. being a travel nurse you will get a lot of the most unwanted patients because they go by seniority. pay is very good
