  1. Sorry if this has been covered, but I haven't done MDS's in a looooong time. Under the locomotion on unit, if a resident is pushed in a wheelchair, what do you code it? I have looked back to see what has been previously coded and sometimes it is limited, and sometimes dependent. The MDS 3.0 doesn't give an example
  2. OMG! I think I worked in your hospital. If it is the one I am thinking about, she is one of the reasons I left.
  3. My first travel assignment was in Chinle. It is out in the middle of nowhere. They are very travel friendly- I would say 80% of the entire hospital is travel staff. Be prepared for rural nursing. A lot of these patients don't have transportation or running water. They had midwives that were staffed and worked 12 hour shifts with the nurses. I really liked it, but it was quite a culture shock.

    They have always paid me on time. They even let me know if they haven't received a time sheet. They have always done what they would say they would do. I really haven't had any problems with them.

    I worked for liquid agents. My recruiter was awesome! Great assignment. No complaints.

    I am currently working with them. They are about average. They are not the best I have worked for, but they are not the worst either.
  7. This happened a while back at a small rural hospital (no such thing as vip status). One outpatient blood transfusion wrote a complaint. Something about being at a room far away from the nurses station (only one available), slovenly aid (ok, I am an r.n., and I introduced myself as such, and true, I don't iron my uniform, but it comes out of the dryer nice and wrinkle free) I also found her a fan because the room was warm. Ice water was supplied and vitals were taken as per protocol. She stated she was a prominent member of the community and should have had better care. Another incident was some bigwig from a local bank. He went into dt's two nights in a row before his wife admitted that he might have a drinking problem. (He was admitted with an unrelated diagnosis).

    I am just curious what is being done in other places. I am new to this small hospital and the CRNA put the epidural on a regular pump. I am used to the small locking pumps and am concerned over possible mix up's when the nurse goes to change settings for something else. I wondered if locking pumps were the standard. Thanks!
  9. I am new to this small facility. I informed the doctors that it would be safer to have pre-mixed bags of magnesium than the nurses mixing them. The bags that we are to get are 1000ml bags with 40gms of Magnesium. The doctors started talking about how concentrated this was and I had better write a procedure including the use of a buretrol to prevent overdose during the initial bolus. I was taken aback! The pre-mixed bags falls in line with their current protocol. I guess maybe they are having the nurses mix smaller bags for the bolus, however they don't have a policy stating this. I don't mind having the buretrol as an added safety feature, I was just surprised by the reaction. What is everyone else doing?

    I really should know this, but luckily I have never had a patient with a cord prolapse. When they tell you to put pt. in knee-chest, do they mean like a McRobert's maneuver where the pt. is on their back and the knees are flexed up on the abdomen, or is the pt. suppossed to get in a modified hands-knees position. (I always thought that one would be kind of hard to get them to do when you have your hand up there.) Thanks!
  11. ((((((((((((((((((((soul))))))))))))))))))))) Big hugs to you. Been there, done that. I too am quiet by nature. I had someone training me who had been at that hospital for about 23 years. If things weren't done her way, they were wrong. Different nurses have different styles. You need to be able to learn in a supportive environment. Not one where you are nervous and jumpy because of your preceptor. Don't be so hard on yourself. You will get it! Hang in there. Oh, by the way, I left that job and became a travel OB nurse. The hospitals where I worked thought I was wonderful, and it was a great boost for my morale!

  13. My DON handed me a complaint to respond to. A "respected member of the community" had come in as an outpatient for a couple of units of blood. I escorted her and her entourage to the only available room, got them all drinks, and a fan because it was a hot day. I started the I.V., gave the blood as ordered and monitored her per protocol. Her statement said "I was taken to some room all the way down the hall by some slovenly orderly (I am an R.N. and she was told this and although scrubs aren't all that flattering, I have never been called slovenly!) Anyway, she went on to say that she was ignored way down there at the end of the hall. Did she not notice all of the other patients on her way down there?
  14. I had this surgery right after I turned 30. I had c-5 & c-6 fused. Never had the cortisone shots though. The surgery was the best thing I could have done. I had weakness down my right arm and my right thumb was numb. The reason for my surgery was stenosis caused by a bone spur. I have full range of motion in my neck and full function and feeling back in my right arm. If your brother has any numbness or tingling I would be very wary of seeing a chiropractor.
