Falls
Check the residents diagnosis list. We had a resident with frequent falls recently. He had a diagnosis of BPH on his list, so we got the MD to order a PSA. Turned out that his PSA was 3 times above the normal limit. A good rule of thumb---always look outside the box!

sick and tired of defending working in geriatrics..
I have worked in both settings, hospital and LTC. LTC is more stressful and much more rewarding. In the hospital, you deal with the acute situation then send them home, or back to the LTC facility. It makes a big difference when you have taken care of someone for several years, watched them go from walking the halls to being bedridden and then sit with the family while they are dying. You try hard not to get attached, but it is impossible. I have no desire to anything else but work LTC. I work 12 hour shifts and it is non-stop all day.

the little things make you go uh ok
Had a patient in ER once who said he was allergic to Advil but he could take ibuprofen. Explained that Advil is ibuprofen, not sure it sunk in. Doc gave him a script for ibuprofen and he went away happy.:lol_hitti

Roll Call: Introduce Yourselves, LPNs!
I have been an LPN for 18 years and have worked in various areas, thanks to working for a staffing agency. Currently work in LTC, which is where it all started 23 years ago as a CNA. I have worked in hospitals, doctor's office and in administration--always go back to LTC.

Does "the state" visit your facility often?
It is the same here in KY. We have a yearly survey. We are expecting ours here any day--oh what fun!! :monkeydance:

Question about skilled nursing and g-tubes
There isn't a SNF here that doesn't accept a patient with G-tubes. The floor I work on has 63 residents, 14 of which are g-tubes. We have g-tube patients on all 3 floors, plus vent patients, trachs, PICC lines, IV's etc. Fortunately, for 63 residents, on day shift (12 hours), we have 2 charge nurses, myself and my partner, and 2 med-techs. And still, on most days, it is a race to get everything done!

What is it with the nurses in LTC?
I have worked in LTC and in hospitals and I agree with the others--you can find attitudes just about anywhere you go! LTC is not an "easy" job as others might think and the stress can be overwhelming.
