  1. I was wondering what Xanthorrhea means? Does it mean a yellowish discharge? Also, what does mycohemia mean? does it mean a condition of fungus in the bones? Thanks
  2. I have been given a case study to complete that I am having a difficult time with. It is not over any information that we have learned in class and I can't find the info in any of my textbooks. Any suggestions or thoughts would be greatly appreciated. The case study reads: This is your first night working actually working as a nurse in a long term care facility. You have already completed training and orientation. During report, you are surprised to learn that you have 14 very ill patients and things seem very disorganized from previous shift. This is one CNA who will assist you. 3 hours into shift you realize that you are not keeping up in terms of passing meds or changing dressings. You notify the float CN and she doesnt offer any help or solutions. 1. What are legal issues that are in risk in this situation? 2. What can you do about the situation? (Answers need to be thorough and detailed)
  3. We are studying diabetes and were given an assignment showing some doctors orders. We are to explain why the order is right or wrong. Maybe I am not understanding, but I just don't feel I was given enough information. I am especially having trouble with #3 on the the list which I copied below. I really would appreciate help, thanks. Physician Orders for the seriously ill Diabetic patient 1. Regular diet 2. Up ad lib 3. Start an IV D5W @ 80cc an hour 4. Measure & Culture wound on Right leg 5. Call results when available (what other info will the physician ask for if any?) 6. Daily dressing change using kaltostat & cover 7. Daily weight 8.   4. Thank you tampagirl. So, I am correct in the diagnosis being risk for fluid deficit? I have looked through the nursing dx several times and I keep thinking there must be a more fitting one, but maybe I am just not getting it.
  5. I am trying to do a careplan for a postpartum mom having a risk of hemmorhage. Would the nursing diagnosis be risk for fluid deficit? And what could I say it is related to? Thanks in advance for any help!
  6. I cared for a newborn in my first day of mother/baby rotation. I have to complete 2 careplans for this child, however there was nothing out of the ordinary. Baby was doing fine. Any suggestions would be greatly appreciated

  8. I have an assignment where I have to compare the approximate cost for Foley catheter insertion. I have to compare a hospital's charge for this, a clinic's charge, and a nursing home charge. I cannot get a quote from our local hospital or clinic. I obtained the nursing home info. Does anyone have any idea on the charge?
  9. Thank you all so much. I knew A & D were wrong, but had debated back and forth between B & C. I was just thinking about this and wondered if the Dr. actually orders the flow rate on continuous irrigation or can the nurse adjust it based upon the color of the output. I read in my book that it should be pink to light red and that the color would fluctuate based upon the flow of the irrigation.
  10. I thought that bladder irrigation after prostate surgery was to remove clots in the bladder and prevent oozing of blood. I have a multiple choice question that I am not sure of. It asks the purpose of continuos bladder irrigation after prostate surgery. My options are: a. hydrate the pt b. dilute the urine to stop clots from forming in the catheter c. stop bleeding in the bladder d. promote clot formation I think the answer must be c, but I can't find anywhere that says the irrigation actually stops bleeding. Any help would be much appreciated.

  12. I have a question in my workbook that asks: The Dr. orders throxine 0.2 mg p.o. for the pt. Throxine tablets are 100 mg each. How many tablets would you give? I am not getting this as the tablets are 100 mg and the order is .2 mg. Any help would be appreciated very much!

Draw a sensory neural pathway from the skin of the foot to the appropriate cerebral cortical area. (Refer to the homunculus for locating the foot sensory area.) Be certain to include three neurons with a cell body, dendrites, axon and axonal terminus on each neuron. 2. Draw a motor pathway to the muscles of the left hand using the same rules outlined for the sensory pathway. Be certain that the axons of neurons travel vertically only in white matter. I am lost and so are my classmates. Any help would sure be appreciated!

  16. this is my rough draft- any suggestions?? i was astounded by the news i had heard. i had worked for a telemarketing company for 9 years and was planning to stay with them until i retired. now the chairman of the company had informed the department heads that the business was shutting down. i felt disconcerted. not only did i have to inform my employees that they would soon be out of a job, i also had to prepare for my own loss of employment. i didn’t realize it at the time, but this was a true blessing in disguise. shortly before being informed of my pending job loss, we had placed my grandmother in a long term care facility. i was spending considerable time there and was attending various activities with her. i really enjoyed the residents and the caring atmosphere. therefore, i decided to talk with the administrator about the requirements of an activities assistant position. he informed me that the first step was to get enrolled in a certified nursing assistant program. he said that after obtaining my certified nursing assistant license, i could work as a nursing assistant until such time that an activities position became available. consequently, i decided to take the certified nursing assistant course in preparation of a position in the activities department at the long term care facility. i accepted a position of a nursing assistant and began my employment there midway through my nursing assistant class. i found that i really enjoyed working as a certified nursing assistant. i appreciated the opportunity it provided me with for one on one involvement with the residents. i soon realized that even though this wasn’t my original plan, working in the nursing field was ideal for me. nine years later, i am still working as a certified nursing assistant for the same long term care facility. the residents there are like family to me and i have wonderful relationships with them. i never knew what a positive impact the residents could have on my life by nature, i am a sincere, dedicated, caring, individual who has a great deal of ambition. i love to learn, and am always up to a challenge. i prefer to be in a setting where i can interact with others, although i can efficiently work on my own as well. i have the ability to effectively communicate with people. i understand that each individual is unique. through my employment at the long term care facility, i have learned to adapt my approach to meet the needs of the individual i am interacting with. i have also thought the challenges i might need to overcome as a licensed nurse. i think the biggest obstacle would be delegating responsibilities for the residents. i tend to prefer to do things myself, so that i know that they are done to the best of my ability. i am confident that this is something i can use to my advantage. as a licensed nurse, there are many different ways that i could share my passion to provide the best possible care for the residents. after much consideration and research, i am determined to continue my education in the field of nursing. i have spoken at length with nurses at the facility where i am employed and i have observed the charge nurses with their responsibilities. i have an accurate idea of what nursing involves and i know that i would be extremely proficient as a licensed nurse. my experience with the nursing assistant program offered through ---college was wonderful. ----was a very compassionate, knowledgeable instructor. i have also talked with nurses who have completed the vocational program through --- college. i am convinced that this nursing program is the right one for me.
  17. I am applying to a LPN program at a community college. One of the requirements is a personal essay explaining: 1. Why you want to be a LPN 2. Why you should be admitted to the nursing program 3. What are your strengths and weaknesses I am having difficulty determining what format to write this in. Should I write it like an essay or just the facts? I feel like I should start it off with something about why and how I became a CNA. The telemarketing company I had worked at for 9 years was shutting down and I was lost. My grandmother had recently moved into a nursing home that I felt really good about. In talking with the administrator of the nursing home and the job service center, I learned of a program for displaced workers and decided to take the CNA course. The closing of the telemarketing company was a blessing in disguise as I love being a CNA and working at the nursing home. The residents there are like family to me and I have wonderful relationships with them. I never knew what a positive impact the residents could have on my life. Is this something I could start my essay with? If so, what should my opening statement contain? Any suggestions would be greatly appreciated.
