Burnout?
It has been my observation that our patients and their families have been seriously behaving badly like spoiled, rotten, I, Myself and Me first kids.After 25+years in adult critical care,I have had it! Acute bedside nursing has robbed me of my love,caring and compassion for the sick.Our poor nursing students have nonsense requirements that in the end,they will barely use at the bedside where they(the new grads) are needed.Also,what is up with the other members of the healthcare team? Why is it that they try to dump their jobs on us? PT gets the pt up and leaves them in the chair and they don't come back to get them back to bed and expect nursing to do it.Case Managers that ask us to call the docs to ask them to transfer/change the pt status when the doc's notes clearly says will stay overnight(mind you this pt is fresh post-op). An nurse admin who says that every nurse can precept a new grad to ICU and no, they will not get any extra compensation because"I believe that it is every nurses' responsibility to orient/precept".And many more irritating mindboggling nurse dissatisfying stress-inducing instances that occur rather too frequently lately!!!!!!Grrrrrrrrrrrrrrrrr!!!!!!! I am so done with bedside nuring and the Bs that go along with it.I want my sanity back.Yes,I have a new job and love it. At this point, money is not everything. To everyone,take care of yourself first so you can continue to care for others.

Amiodarone IV: needs an inline filter??
Filter is used for both Amiodarone and Dilantin. Usually the filter size is 0.22 micron. Dilantin is mixed in Normal Saline only. Lipids are never filtered and if your facility uses filters for their TPN's then the Lipids go below the filter or the port closest to the patient. If in doubt, call your Pharmacy or use your drug books or Micromedex (or the likes), because Zashagalka is right about being a prudent nurse. Also, I do not understand why some nurses readily forget their past life as a newbie.Good luck!

Death by Arterial Line?
Point #1:Invasive lines are a bold invitation for infection; Point #2: Exsanguination requiring blood product replacement is a big SENTINEL EVENT that should never happen anywhere. Point #3: Always have alarms on for art. lines in place.Use alarm parameters as Systolic,Diastolic, and the current practice,MAP. Leaving any invasive line for the convenience of the staff is a no-no. This patient is not on pressors,not a hard stick,etc.The line must go! Just my 2-cents. Good day!

Come work with us here in SunCity West and or SunCity.Sunhealth Corporation is it.Have two hospitals mostly geriatric cases although we have an L&D at Del Webb.These hospitals are just a 5-15 minutes from Surprise. What unit do you want to work at?We also take new grads since we have our affiliation with the school of nursing.Come and give us a try.I have worked at the Del Webb campus for 11 years in ICU. Interview with other Nurse Managers and unit staff is coming up this Friday and I would like to get some examples of questions we can ask.It is a critical care unit and we cannot seem to keep NM's for long.It is a small community hospital with 250 beds.
