Nurse lou unfortunately the state can only make recommendations and hold the hospital accountable when they follow their own Pocily you need to find the policy regarding staffing in the ICU, in a closed unit like the ICU I would expect to see a min of 2 nurses simply because who is going to cover for your breaks that are required by law. This sounds like a compliance issue as well as a safety issue, call your house suppervisor and request to see the policy and make a call to the compliance line.

I think you handled the situation great, but I would have asked her in the later conversation about the headache ie if she was hearing voices etc... she is one Zyprexia but only 5 mg according to your post. Zyprexia in my experience (psy nurse 10+ years) works really well or doesn't work at all, I would however question the 5 mg dose for an adult unless she is very small, she may need an increase in the dosage. psy nursing can be as fun as you make it. the patient are trully a gift to work with, I have learned something new about nursing, life and my self with each encounter with a patient. good luck with your future.
  3. hooray, anyway you get to the RN is great, I did sort of the same thing but it is the end result that counts don't pay attention to the nay sayers about getting your LPN first, they don't know your situation and so their oppnion does not cout in my book
  4. first you need to take a deep breath! slow down a bit. if your are feeling like that nurse was watching you and going back to your instructor then you need to foster a relationship with her. ask her to help you out when trying to figure out all the lines. to give you some pointers when caring for this patient etc... then Never put something on the care plan that you do not intent to do, review it often to make sure you ARE doing what you said you will do and review it agin when you are documenting the care given to make sure you documentation reflects what you did, you might want another student review your documentation as we often do not catch our own mistakes. The fact that you are ADD should not matter but YOU do need to learn to focus, and carry out patient care systematically with one patient or you will never be able to manage several patients at a time on the floor. and finally pick one care plan book and make it your bible. individualize the plans DO NOT use verbatum from the book unless you can verify it fits. And do not address problems that you can not do something about.
