  1. The main information administration looks for on the forms is "adverse patient outcomes". We also have a bargaining unit facility, and have used these forms for many years. They are looked at every month at our labor/management meetings, and the ones that get the most attention are the ones with documented patient/family complaints or adverse outcomes. We were able to increase staffing on one unit because of the large number of forms the nurses had filed with management. Hope this helps.
  2. Along the line of the lady with the BKA, about 15 years ago, we had a patient admitted from ER with CHF and huge, swollen legs. The family had been tired of changing his bed sheets at home because of the fluid seepage from his legs, so they made him sit in a chair with his legs in a garbage bag. Yep, you guessed it-the squad that brought him in told us this was the absolute WORST patient they ever saw!!! The family told the ER staff "We change the bags when they get full!!!
