This is how I did it: I sent the form to my school from there, and my school sent the state and some paperwork, or my transcripts, I guess, to Iloilo CHED, then CHED sent everything back to my school. I had my mom pick it up, and she processed it at DFA for apostille. After the process, my mom sent it to the board.

Hi, yes, you still need to process that form. They just need to verify your diploma and that you graduated with a BSN.

In my case this is what happened: I had my mom drop off the form to my school. My school processed it and sent it to CHED Iloilo. CHED processed and sent the papers back to my school. My mom picked it up and sent it to someone from Manila and this person made an appointment to the DFA and processed my papers on my behalf because my mom is not from Manila.

You have to send the form to your school from there they're going to send it to CHED for authentication and the CHED will send the papers back to your school and then your family member will pick it up and send them to DFA Manila. I believe it has to be an in person appointment at DFA.

I took my exam yesterday at 8am and yes this is my first attempt. I really hope and pray that the PVT was wrong and that I passed.

@coffee, just keep on studying. I was torn when I did the Pearson vue trick and I actually tried to change the CVV number it said incorrect and tried to put incorrect credit card number as well. So I told myself OK I assume I failed, so I just went ahead and paid the registration of $200.

@coffee. I used Rachel Allen review center, q banks, also did some Kaplan reading. I think I have to submit a “Reapply for Exam” application online besides paying $200 which I’ve already paid ?. So I’m just waiting for official result and maybe purchase a quick result once it’s ready so I can move on with the process again.

I live in California how about you? In ca.brn there’s an option for “reapply exam” you didn’t do it? I’m confused if I should contact my brn. I’ve also paid $200 when I did PVT and went through so I’m assuming I failed at 75q. thank you for your reply.

Hello, to the ones that had to retake the exam. Did you register to Pearson right away and not contacting you BON for retaking the exam? because they said I have to contact my BON. But I went and registered again and paid $200 , I also sent an email to BON that I am retaking my NCLEX RN

You may also want to check Google—type in the form's name.

The form is on the BON website. Type the name of the form in the search bar. It should pop up. If not, your nursing analyst should've emailed it to you when they wanted you to process this form. All the documents are on the BON website.

Hello everyone. The BON approved my NCLEX Application. I am now waiting to register at Pearson VUE, Pay for registration and get my ATT.

Not sure who else needs that form. But I applied for the initial exam for NCLEX RN. My papers are on the way to CHED now. It's a waiting game.

I am not a registered nurse in the Philippines. I received an international diploma verification form as well. I'm confused because I thought that form was only for registered nurses.
