Hello everyone! I'm an RN currently working in outpatient endoscopy, and I recently got a job offer for pre-op/PACU at a multi-specialty ASC (ortho, gyn, urology, plastics, ENT, eye, etc.), both adults and peds. I've been in outpatient endoscopy (pre, intra, and post) for two years, and I worked in med/surg for about a year and half before then. Here's my concern. Recovery from propofol sedation in endoscopy is super easy. I'm a little worried about learning to work in a "real" PACU with general anesthesia, including for peds patients. Does this sound like something that would be safe for my license and my patients? For anyone who has ASC experience, I would love to hear about the transition/learning curve!

Hello all! I have a question about drips/compatibility. I had a patient on a heparin drip. The patient was a very hard stick, only one PIV. Prior to initiating the heparin drip, he was only receiving MIVF (NS). Since NS and heparin are compatible (per pharmacy/resource), is it okay to y-site the heparin into the NS? (two separate pumps, of COURSE). There is no protocol stating that heparin needs to be run completely alone. It seems that this would be okay, but y-siteing the NS into the heparin would NOT be okay since that could result in bolusing the patient with the heparin between the y-site and the patient when first hooking up the NS. Am I thinking about this correctly? Different nurses had different perspectives on this, so I would love to get some clarity!

Hi, @brume! Congrats on making it to third semester! I'll be starting at Duke in the fall, and I was wondering whether you could tell me about the difficulty of the program. I've heard horror stories about ABSN programs and how hard they are - I'm getting so nervous about the whole thing! Is Duke insanely hard, or has it felt manageable so far? I've also heard that lots of programs seem to try to fail their students (or take a "weed out" approach) - have you found that to be the case at Duke? Thanks so much!

Thanks for your reply, byunowho ? That's good to hear. If that did happen, I guess I'm just worried about Duke potentially finding out and then rescinding my acceptance. Sounds crazy, I know, but I've heard of that happening with undergrad!

Hi, everyone! I'm anxiously awaiting my decision, but I had a question. I was accepted to another program, and I have to put down my enrollment deposit by next week if I want to reserve my spot. However, Duke is my first choice! Is it okay to commit to the school that already accepted me but then rescind my enrollment if I end up getting into Duke? Basically, I'm wondering whether this could create a problem (besides losing my enrollment deposit)... I would greatly appreciate any thoughts!
