You also should provide your entity number. If you don't have it, give them a call. At first they couldn't find my file and I've gone mad! Lol! I thought my $750 application fee went down the drain for nothing. That was why i drove to sacramento. The reason was when i re-applied, i used my married name. My test results were under my maiden name.

I sent a link to the notification of name change form but it won't go through. Allnurses thinks it's a spam. But anyway, go to CA board website and look for that form. Send it via fax or mail it to them. Address and phone number are there. It shouldn't take long before you get yours!

Hi jaides!! Oh i know how scared you must be feeling! I drove to sacramento when i saw my application was cancelled! But don't worry, they just need you to sign a form regarding change of name, copy of your SSN, ID and copy of marriage certificate. I got my license last month. Online, my application is still cancelled but i have my certificate with me already.

It takes longer for some people like me who's been waiting for a decade now for my license but I'm positive it's worth it. Thank you so much for this thread. It helps me a lot! Hi! I requested the TOR from my school but they dis not send the RLE. What happened to yours? Did you ask the school for the RLE? Did it cause delays?

My school submitted the transcript, breakdown of educational program amd program verification from the Dean. However, they did not send the RLE. Now the school wants me to request next week, then wait til it's completed and sent to the board and for the board to process it. I think mine will take so many months. There are so many delays I'm so frustrated now! But so happy for you after a long wait you'r licensed already!

I submitted my pass letter, marriage certificate, birth certificate (although not necessary), copy of diploma, copy of Philippine license, the live scan, check amounting to $750 this June. They cashed out the check Aug. 3rd. I created a breeze account as well. Recently, my pending status became "cancelled". I called them and they couldn't give me an answer so I drove 2hrs to sacramento just to tak to someone. She said it's because when i passed, i applied using my maiden name and now I'm applying for my license using my married name. Until now my status says cancelled. I received a mail asking for a copy of my SSN last aug31st, which I promptly sent. They were also asking for the transcript, breakdown of academic something and the RLE. I'm not sure if the school also sent the RLE. That i have to ask. But the board received the forms sept 5. As of now they said they haven't received anything and for me to wait 12-16wks. It's so frustrating..

Congratulations! Took you a year and more to get your license?? Oh my! That's a long wait! I submitted mine this June, they cashed the check out in august. Aug31 i got an email regarding confirmation of email. They received my transcript according to tracking number sept 5, called in today for update they said wait 14-16wks more. What caused your delay?

Hello! How long did it take for you to get your license? I'm having the same experience. Took mine in 2008 and i only had my SSN this year. I reapplied for examination last month and today they processed my check. Please help! Thank you
