if you are a nurse who did physical examination in a community. Who would need an immediate follow up :

I'd have to say the 3 y/o boy

Definitely the 3-y-o -- major red flag for autism. You'd want to tell the man to have his BP checked again by his PCP within a month or so, but it's certainly not high enough to warrant immediate follow-up.

At first I was thinking - 3 y.o. old autistic boy but than the rational for most priority questions is that you act on acute illness first and then the chronic one. The blood pressure systolic 150 put him in Hypertension level 1 and risk for stroke. SO, I wasn't sure.

Tough one. 150/88 isnt that high to cause stroke any time soon. Yes its elevated blood pressure but you know its not that dangerously high.

Not only autism but all kinds of other problems like abuse, etc. I would say the boy too.

With my limited knowledge, I was going to say the little boy because of autism, abuse, etc. Like Tweety said. Sooner he gets into treatment, the better. The man's BP was not dangerously high but should be rechecked soon as BP is dynamic... it could have been high for a number of reasons, recent stress, "white coat syndrome" for examples.
