Hello all!
I'm just wondering if there is a standard PRC Scrub Form? I've seen that some other schools have their own format of the said form and sometimes it's just become way too over--being so time-consuming and some details are unnecessary.
I hope the PRC will implement some "official" format for the benefit of the students who had to "reprint" their forms in case something was changed...

Well, since PRC has not implemented a template for this, you may "use" the format of your predecessor. That is, the previous batch. Or you may ask your school for a format that their students have been using since then. Don't rely on other schools' format. instead, stick with the complete format of your school (but you have to be keen if there are any changes made ok). Then, you talk to your school/batchmates, that you stick with that format so everything will be ok. Hope this helps.

in my case, when i was about to apply for nle, i was really worried about the format. so many hearsay and i keep on changing my format. i was almost ready to give up. we even changed paper size because some says it is required... blah, blah, blah.. and when i get to prc, they just looked at it and it was ok. as long as you did all ur cases, everything will go well. the prc are just suspicious to others who didn't do their cases for real. just don't use abbreviations on diagnosis and the dates and thats it. follow ur school format as there is no official format from prc. i hope they would release a format so others wouldn't have a hard time.

When we were graduating, our school provided us with the PRC Memorandum about the OR/DR cases and that memorandum included a sample format. I suggest you ask your school about it or better yet secure one from the PRC.

actually there is no official format.. ask your school about it for the PRC update them on what are the things neded for the application for the NLE..
