Hello everyone,
Please guys I need your help with my assignment which requires to interview five sources (nurses) to read my definition of the concept of self-management and a model case that represents self-management behavior, then answer this four questions below.
The concept is: Self-management
THEORETICAL DEFINITION:
Self-management defined as the tasks that an individual must undertake to live well with one or more chronic conditions.
CASE EXAMPLE (MODEL CASE):
Mr. Omar is a 47-year-old Teacher, who suffered from chronic, End Stage Renal Disease ESRD. Omar is independent and lives alone. He started Hemodialysis last year when he was diagnosed with ESRD. Because one of his students who was suffering from the same disease five years ago, Omar read many educational brochures and watched some videos about this health condition. When he was diagnosed with ESRD, he asked to be scheduled for dialysis sessions any time afternoon in order not to confound his work as a teacher with his treatment plan. He was an effective participant with the Nephrology nurse and he continues seeking more information about his condition, attending group meeting, and actively involved in his own care. He met once every month with the dietician to discuss his dietary record. He also maintains effective communication with all his caregivers and some expert ESRD patients - who had successfully managed their diseases.
He also has more knowledge about the health benefits of the self-management of his condition. Thus he was seeing a Nephrology nurse to teach him how to perform, monitor, and administer many aspects of his own treatment, including the exit site, taking medications, following diet and fluid limits, and conducting self-assessments of vital signs, weight and observation for complications. Those meetings were crucial for him to understand that the only key to succeed and be healthy was in his hands and his compliance with self-management behavior. He told the nurse, 'I know it will be hard to comply with the treatment plan of ESRD patients, but I have done hard things before and I know I can do it'.
Justify WHY the above example demonstrates a model case:
This case portrays all defining attributes of the concept of self-management. Mr. Omar demonstrates high level of knowledge about his disease, and the ability to monitor and care for himself. He also was able to make decisions and set goals with confidence for the benefit of his health.

You aren't asking for help, you're asking us to DO your homework for you.
An interview for a school assignment requires you to meet with a live nurse and....well...interview him or her. Providing a list of questions on a message board and hoping that it's actually nurses who answer you--and complete your assignment--isn't the goal of your instructor.
Do you know any nurses? If not, contact your local hospital, get ahold of a nurse recruiter or someone in HR (who probably knows which nurses are most likely to help you) and go from there.
This isn't the way to do the homework, sorry.

Most interviews are best conducted face to face. You cannot read tone, facial expression or nonverbal cues online. Plus you have no way of knowing whether someone actually is who/what they say they are--they may not be a nurse but just someone who pretends to be one online. Are you certain that your instructor wants you to solicit responses from an anonymous message board?
I'm not really clear as to what you are looking for in response to your questions. Is the definition your own or predetermined by the assignment? Is the justification your interpretation or that from the assignment (it appears that you cut and pasted the post from another document).
Just as an FYI, if you are not aware typing in all caps is the equivalent of yelling (though I suspect if you are really desperate for answers you might want to yell to get attention. :) ) Is this perhaps due shortly?
That said you would probably do best to interview nurses in person rather than anonymous individuals on an internet message board.

do you have the opportunity in clinical rotations to interact with nurses? you cannot rely on the people here being licensed nurses. you really need to try to interview someone in a face-to-face or a telephonic interview...someone you can verify is a nurse.
the only comment i have, because i don't do homework here, is you should go back and carefully proofread your paper. it has some errors that need to be corrected.
