I've failed my NCLEX twice, and I'm trying to take it again but the Nevada State Board of Nursing won't let me until I send then a plan of study.

Procedure for writing examination after initial attempt

1.  An applicant for licensing as a registered nurse or as a practical nurse may write the examination four times.

2.  
If the applicant is unsuccessful on the second attempt, before he or she may write the examination for the third time, the applicant must present to the Board a plan of study, acceptable to the Board, for preparation to write the examination.

3.  If the applicant is unsuccessful on the third attempt, the applicant must repeat courses in nursing theory in a nursing program approved by the Board before he or she may write the examination for the fourth time.

If board staff approves your plan, you must do the following:

a. Submit a new application and fee to the testing service. This information is found in the
NCLEX Candidate Bulletin,
which can be found at
www.pearsonvue.com/nclex

Does anyone happen to have a format for a plan of study? Or has anyone submitted one before?

How the cover sheet is supposed to be?

Who's it's addressed to exactly?

hi kc 1992, BSN....i'm in the same boat like you. were you able to get a format for the study plan? can you please let me know how you did it? thanks. you need to submit study plan to:

The Director of Nursing Education at [email protected] to present and discuss your study plan with her. The Director of Nursing Education must approve your study plan before you are allowed to test again.

I did not find a format but I just described my plan of study as detailed as possible (which reviewing materials I am using and how many hours a day of studying I am putting in) and I mailed it to the Nevada Board of Nursing.

A couple days ago Catherine Prato e-mailed me asking me to e-mail her my Plan of Study so I did and she told me to e-mail her once I feel confident enough to pass the exam, only then will she approve my plan of study and then my ATT will be released.

hello again kc_1992 bsn. thanks for your reply!so basically, just sending a letter with detailed plan of study is enough to get another chance for att. but it is up to the candidate when to say you are ready for it. i'm thinking of applying to another state like hawaii (they have unlimited attempts for the nclex). let me know when you are going to take the test. thanks & goodluck...

hey! sorry for the late reply, yes she did approve me once I notified her I thought I was ready to take it & my ATT came right away! & I emailed the director of nursing education

I didn't really follow a format...I just explained what reviews I am taking how many hours a day & questions I was doing, just a very detailed plan!

once I emailed her my plan, she told me to inform her when I completed my plan of study & when I did finish it & informed her she approved it & I received my ATT right away!
