Order Set Approval

Hi,

Our Hospital is currently in the process of preparing to go LIVE with CPOE. We have Order sets available, about 150. And we just recently contracted with Zynx to add EBP to our current sets and new ones that will come about. About 3 years ago we there was an order set committee that was charged to develop about 25 order sets to meet the main diagnoses of our hospital. The committee met every other week and was Physician led. When finished with the task of these order sets being developed, the committee dissolved. Currently, our practice is, all e-mail based and then once all parties "give the okay" the order set goes to a formal approval committee meeting. These meetings are monthly. It works but it needs to be better, especially since we are implementing Zynx! Please share on how your hospital approves order sets, any information would be helpful! What is your process? Who is involved? What is the time frame for updating current order sets?

There was just a discussion thread about this on the ANIA-CARING listserve. You might consider joining and accessing the archives for content. Sounds like your organization was on the right track with the formalized order set committee. BIG MISTAKE to ever let that disband especially in light of your plans for CPOE.

I have assisted several organizations in forming order set committees and structures. I can share examples of several different models. The organization I am supporting with their CPOE go live currently utilizes this model:

order sets are submitted from each specialty via a physician representative who also sits on the Medical Informatics Committee. Once content is approved by the specialty, submissions go to the Order Set Committee. This OS committee reviews for things like are the drugs in the formulary, is there any contradiction in the evidence, any glaringly incorrect practices being endorced and to make sure there are not 5 individual hip order sets for each 5 orthopedic surgeons, etc. Once OS approves its off to P&T and Medical Practice Committees then into production for paper form printing soon to be into production for CPOE.

Also assisting this organization in the order set vendor selection process prior to their CPOE Go Live.

Thank you for your reply! Currently I am researching best practices and reviewing the literature in regards to CPOE Order set processes. I would definitely be interested in seeing more committee structure and policy. I will send you my e-mail contact information. Thank you for letting me know about the ANIA-CARING listserve, I plan on checking that out! I agree, letting the order set committee come to an end was not beneficial. I have only been in my role since the beginning of the year, and knew nothing of informatics and picked up on this right away! It's been a lot of "clean-up" the past few weeks! I look forward to hearing from you more.

we have zynx, the nice thing is once you built an order set, you can send it to the persons you want to review and they can put thier comments on them.

we have a committe which include doctors, quality, informatics, & pharmacy they go thru and approve and then present it to the speciality physician that it pertains to. Our order sets are diagnosis or procedure based.
