Obtaining scripts as an indepedent contractor

I currently have been asked to provide evaluations/checkups for some group home residents (working as an independent contractor). How do I go about obtaining my own script pads to write prescriptions?

    you need to contact same place who issued you your current rx pad. and have a new Rx pad issued for place where you work.

    In NYS it would be NYS Dept of Health

    you may want to contact state BON and/or state Dep-t of Health. In NYS it is DOH that issues them.

    perhaps ,you can ask other NPs or contact local NP chapter for which organization issues it.
