   you may want to contact state BON and/or state Dep-t of Health. In NYS it is DOH that issues them. perhaps ,you can ask other NPs or contact local NP chapter for which organization issues it. good luck!

   you need to contact same place who issued you your current rx pad. and have a new Rx pad issued for place where you work. In NYS it would be NYS Dept of Health
