Nurses Pass Ideas?

I would like to start a nurses pass for the teachers/students to fill out. My schools are PreK-5th grade. Right now I just put their names on a sticky note with small details but that isn't working well for me. Send me what you use or any inspo is much appreciated!

I looked up several different passes/templates online and made my own based on the specific frequent needs of our students. For example, we often get hungry children sent to the clinic who are just complaining of a stomachache to their teacher. So now there is a section on the new passes that asks if the student has eaten if 'stomachache' is selected. I would look at some different examples and take from them what you need, (if you are not finding an already perfect pass for your specific student population online or here to use)
