there are actually two parts to your question. (1) how to create a care plan for a subdural hematoma, and (2) how to create a concept map.

to create a care plan for a subdural hematoma you need to look at references on a subdural hematoma. specifically, look at the signs and symptoms of a subdural hematoma. all care plans are based upon the abnormal assessment information you obtained from the patient. this abnormal assessment information is the patient's signs and symptoms that will become that basis for your choice of nursing diagnosis, goals and nursing interventions.

the concept map is nothing more than the physical format in which you will present this information. there is information on care maps on this sticky thread in this forum:
