What are your units using? We use the Babylog with the prongs from hell.

We used to use it for CPAP too until we got the Aladdin and I thought, "Thank goodness!" Now we re extubating earlier and the kiddo usually ends up on NIMV after failing CPAP so we are using those awful prongs more often. Is there a better machine out there?

We almost exclusively use Vapotherm now, up to 8 LPM.

Usually if they fail Vapotherm, they just get reintubated. Once in a great while, we'll try cycled CPAP. It's ancient - we use Neotech nasal prongs, about 5cm long, and Sechrist vents. They don't even MAKE that vent anymore!!!

Gompers, we have one Vapotherm and they are so stingy with it!

We use these prongs that are rubber, but are 3cm long! Back in the day we had a couple of kids loose their septum to it. I can't beleive we are falling into this trend again.

We are using Vapotherm more and more, and WE LOVE IT!! My primary pt and her twin were both extubated today after being intubated and on vents for 41 days, and they are doing beautifully!! They just seem to settle in right away, rather than using all of their energy fighting the CPAP and trying to wiggle out of their prongs and masks. Right now in our 38 bed unit we have 10 Vapotherm units running. We would have more running, but we are out of them right now.

We NEVER use CPAP anymore....[i'm not even sure what Nimv is] Our newest Neo got us about 20 Vapotherms. We use them ALL the time. We recently got a "SiPAP" unit to pilot trial. It's ok, basically a BiPap for neonates, with 2 levels if pressure support synched with the respirations. The cannula prongs are huge, though, and you have to use the old mask. Alarms are kind of pesky, when the baby wiggles the prongs out, and you lose MAP. So we love our Vapotherms. Babies love them too!
