Need to brush up on documentation...

I'm wondering if anyone knows of some good websites that offer assessment and documentation guidelines for nurses. At my private duty job we are given some blank nurse's notes and a vague assessment sheet and little else to go by, yet the agency is complaining our documentation is not thorough enough.
