about mannitol..

Hello and Good day! Pls help me with my assignment, the question is this... why is morphine not given or administered to patient with pancreatitis? even my clinical instructors are debating about this, they said it does something about sphincter of odi?

Featured Replies

If you google mannitol and look up the contraindications/side effects, I'm sure you will find your answer.

because it was believed to cause spasms of the spinchter of odi which may eventually cause pain.. demerol(meperidine hcl) is the drug of choice..

just reviewed my gi notes 3 weeks ago... hehhehehheheh

Is this about morphine, or mannitol?

Mannitol is for ICP, not pancreatitis, and morphine causes increased muscle spasms in cholecystitis- not sure about pancreatitis.

From my textbook:

"The use of morphine was avoided in the past because of concern that it could cause painful spasms of the sphincter of Oddi and worsen pancreatitis. However, there is no evidence that this affects the outcome of the disease. Meperidine (Demerol) had been the medication of choice, although all opioids stimulate the sphincter of Oddi to some degree. There is no clinical evidence to support the use of meperidine for pain relief in pancreatitis, and, in fact, acculmulation of its metabolites can cause CNS irritability and possibly seizures. The current recommendation for pain management is the use of morphine (Reddy & Long, 2004; Swaroop et al. 2004)."
