I have read about the indiana case where the nurse accidentally gave too many milligrams of some drug to some infants.

I have also read that the medicine cabinet was stocked incorrectly.

However, the nurse is still ultimately responsible with giving the injections, right?

There was more than one nurse involved in this incident. Also a pharmacy tech. Yes, there has been a lot of discussion on this. I live in Indianapolis, so have been hearing about this daily.
