I just got my independent provider number for nursing through the Ohio medicaid waiver. Im totally confused. In one place i read that an independent can not provide services less than 4 hours. It has to be done through a home health agency. But then I see requests for IP's for one to two hours only. I also can't find any resources to help guide me on what to do once I get a consumer. I heard that I need to fill out a 485 and have to submit it to he DR. But then what? Just start providing services? Also, do i fill one out for every new client? Please HELP me!!!!

485 are for Homr health agencies not INP.
