IHP for amputee?

I have a student returning to school on Monday after having a total foot amputation last week. Do any of you have an IHP that I could review (with identifiers removed of course) for an amputation? I googled IHP Amputation but came up empty handed.

I ended up tweaking a spina bifida IHP because of the limited mobility similarity. My fellow nurses say it's great so yay me!
