hey! so i'm a level 2 nursing student and my best friend is a level 1. she just submitted her first care plan and her teacher completely tore it apart saying it was horrible! i am trying to help her revise it, but i thought i'd see if there are other people who could lend advise as well.
here is her nursing diagnosis:
excess fld volume r/t increased abdominal grith s/t extra vascular ascites
her aeb is:
procedures pericentsis, clear fld draining out of the abdomen
it's only her first one so it's not perfect but she needs a little help...thanks!

Now would be a great time to invest in at least one care plan book - I personally use 3-4 since I don't want to "copy" straight from the book....but they saved my butt when I first started careplans - especially since they were so foreign to me....mine bled red ink! there are some really good, helpful nurses on here that can help with care plans...might want to revise your title to include careplan help. Good luck!

This is something she will have to figure out on her own. It's the only way. Your friend needs to find a resource nursing diagnosis book and remember her pathophysiology and knuckle down. We all had to do this type of homework. It is hers to work through as well on her own. Finding her own way to her careplans will give her the most knowledge.

Is the instructor willing to explain what it is missing, and what the instructor would like to see? This kind of guidance is what helped me improve my careplans. I've noticed that expectations vary from instructor to instructor a little. Good luck to your friend.

I'd recommend Understanding the nursing process by Carpenito-Moyet and All-In-One care planning resource by Swearingen. I've used these throughout nursing school and have always (except the very first one) done quite well with my care plans.

it has every thing I think the diagnosis itself needs to attended to fluid volume excess related to (what ever is causing the excess volume for eg..increased sodium. kindey disease etc) as evidenced by weight gain or edema or as she said increased abdominal girth but an increased abdominal girth may not necessarily be a result of increased fluid volume unless the patient has generalized edema.hope it helps let me know....
