I called that firm too. My situation was different.  I regret not paying..

Any provider that is certified in Addiction Medicine.  That is an addictionologist, but it could be a clinical psychologist or a psychiatrist (psychiatrist is a MD or DO so it holds more weight than a clinical psychologist) but it could be a family practice physician that is certified in addiction medicine.  Your best bet to be safe is to find a Rehab Center and have your evaluation there.  Call a rehab center and ask them if they do comprehensive substance use evaluations or substance use evaluations.  Nearly ALL of them do.  Some don't, but the overwhelming majority do.  Your quickest and safest bet is not to search for one individual provider, but search for Rehab centers.  Go to their websites and under services, you will see where it say comprehensive evals under admissions, and even if it doesn't say it, you can bet the overwhelming majority do them.

One of their psychiatrists. It can't be any psychiatrist or addictionologist. Psychiatrist likely more reasonable.

In that case a board certified addiction specialist would be best.

If you are 1000000% sure they have absolutely nothing on you, then I would 10000000% go for it. screw. them!! 

I wanted to do the same thing, but I had missed two charting on two narcs (or maybe one was back charted I cant remember it right now)  and had already spent $8-10k, so I just said forget it. It takes more money and time to go through the administrative law, and every time they reschedule the date - you'll have to pay your lawyer, unless you don't have a lawyer then go all the way. Still, please make sure to consult with your lawyer or someone who can give you legal advice. That was just my case. They still gave me a one-year probation. I was too scared at the time, now that I'm on the other side, I wish I had been more firm on fighting them but also  MI BON is ruthless.
