Help!!!!!!!!!

Hi everyone !!! I am soooo stuck I would appreciate any advice possible . I am sick to my stomach at the place I have chosen to work and I am scared for my license and reputation working with these people. I am working within a very large hospital in the long term care end(subacute)but we haqve way to many chiefs and absolutely no organization,teamwork or management. The scenario is ,I took care of another team of pts on 3/15 and noted in the chart and put in the Np book that one of the female residents had a fishy malodorous smell in her brief when changed. I also informed the unit manager who responded by querying about the residents shower the day before and saying she just needs a shower. mind you she is post CVA,Gtube,contracted and aphasic. She blew it off and now 1 month later the resident is peeing blood,fever 102.7 and here comes the kicker the note I wrote on the 15 is gone out of the chart. What do i do???????????

Although the pay may be nice, you definetly don't want to work in a place like that at all! If you're not happy with your job, it may be time to submit a request to be moved to a different department, or simply find a new job. Team work and manegement are critical when it comes to hospitals.

Now, you could always try to make a difference. This would start by founding out some way who went back and adjusted the chart last (in my hospital, whoever adjusts the patients chart has to sign and date a paper attached to the records of the patient - so we could easily fix this problem). You could directly question people about it, since there needs to be someone that saw something and is an honest individual. Other than that, I'm honestly not sure how you could find out who changed the chart.

Once you get this turned around and still want to work at this facility, my advice would be to form a group of nurses that want to make a difference as well... form a sense of team-work and take it up with manegement that you're not happy with the way things are running, and think of some way or something you could do to get it turned around.

Thank you antidote for the reply. I have only been there 2 months and suggested some things to improve the system but they are way stuck in their ways and very clicky. I have only been a nurse 1 year and the other facility had a great system I am sorry I moved here and am in this position now. They don't even count the narcotic cards only the pills and have alot of agency that comes through. i am afraid that one day someone may take a card of narcotics and the paper out of the book then its whoevers word they believe. I know the system as a whole is a great one but I am going back to school and until then I do think the best thing is to phase out before I lose my chance at a real future in the big hospital.

If you really want to stay in a hospital setting (which I don't blame you for at all) then my advice would be to let your manager know you're going back to school for an RN and most likely they'll keep you there until you get your RN.

With your RN you'll be able to do a lot more (as you know) as well as even have more say within the boards I believe. Are you going back for your associates degree in science nursing?

Go to the head of the stream (ADON,DON) and tell them your concerns and questions..I don't believe they got where they are by not being good people. Pills - count THE WAY that makes you feel safe - IT"S YOUR license!!!! They can't make you not do it the way you feel is safer - whether or not everyone does it.(Learned the hardway)..

Again Thanks!! I am in the long term care facility of a major hospital. So i don't consider myself in the hospital. I am going to go for my associates . I ahve all my prereqs to do and then the bridge program but I will do it. I just got off the phone with a recruiter for the state prisons which have a great benefits package and tuition reimbursement so we will see where that goes. The manager where I am is the one that I believe removed the notes from that day and the next (1 sheet)and the DON is only 1 year there and not a strong allie. From what I have seen she doesn't like to rock the boat but she needs to rock something,last survey 30+ tags

:idea: I used to fear getting old, now I fear having to end up in a LTC facility that is yuck.

I agree, find another job. Your license is not worth it. I too live in GA just passed boards and really do not want to go into LTC.

Oh hell no. That is NOT cool. I would have a FIT if my nurses note was not in there. Girl, run away from that place and run as fast as you can. For real. This sounds like a terrible place to work.

Do you have 24 hour reports? Did you happen to put it on there? I mean it would be really unrealistic if EVERYTHING about that came up missing.

If I were you I would find another job, it is not worth losing your license over!

Yeah, what about your personal worksheet that you carry around with you? It is INTOLERABLE for notes to be missing. VERY, VERY, VERY illegal. If I were you I would take this as high as you can to whomever will finally do something about it. I forgot what the original problem was, I'm so heated up about the nurses note missing.:angryfire
