there is a link there to the ismp (institute for safe medication practice). i'm not sure if they have an official protocol for a med error or not. but, you should check their site.

http://www.ismp.org/tools/frederickreporting.asp - this is one of ismp's medication safety alerts. most of this page talks about the program in operation at frederick memorial hospital in frederick, maryland. toward the bottom is a list of the "procedures and responsibilities" of the various members of the healthcare team involved in medication errors. i think you will find the information you are looking for here.
