Help with NANDA

I have another one:

Although recovering, T.P. suffered a stroke that has paralyzed his right extremities. He is right-handed.

My response:

DX- Impaired adjustment

R/T- paralyzed right extremities

AEB- patient's statement "I don't know how I am going to deal with my partial paralysis."

Can a patient's statement be listed as an AEB? My instructor said we can make up anything from the scenario that was given. Any advise/feedback would be great!

Both of your examples look very good. Remember that a nursing diagnosis should be one that is related to the patient's response to care or the patient's response to their primary medical diagnosis.

The manner in which you established your diagnosis looks good. Here are some others that might help you think along these lines ....

For your first example:

Alternation in comfort related to LRQ pain as evidenced by patient's verbalization of pain (or you could use your orginal statement) .... as evidenced by patient grabbing lower right side of abdomen.

For your second example:

Ineffective Coping; Individual, related to paralyzed right extremities as evidenced by patient's verbalization of, "I don't know how I am going to deal with my partial paralysis."

Looks like you are on the right track. The problem with nursing diagnosis, is that as students we put way too much into them and as practicing nurses, we very seldom use them. But for purposes of learning, it is a good idea to know how to fit your patient's response to care into a reasonable nursing diagnosis.

Now that I am an actual nursing student I think about my visits in a health care setting and can not recall nurses actually doing some of this stuff. It has been great learning about it though!

Just thought I would help a bit. I would go to the bookstore and look through some care plan books. They outline NANDA format diagnosis very well and then you will get an idea of how these work. Just my two cents. Good Luck!!!!!!!!!!!!! :)

Thanks! I actually found a very good book in the pile that I have collected since last week :chuckle Nursing Process and Nursing Diagnoses. There is such great feedback and advise on this forum!
