can someone please help me how to construct a nursing diagnosis in an easier way! I am having a hard time understanding how to put together a nursing diagnosis for my patient. I have to learn how to do this asap. I have to turn in 2 nursing diagnosis for each medical diagnosis that my patient have. :cry:

I think part of this has to do with knowing what diagnoses are available to you so you can select the correct one. In the meantime, my nursing books talk about appropriate diagnoses for the various illnesses, even for psych patients. Have you checked your med-surg books?

can anyone help me understand how to make up 2 nursing diagnosis for

hypokalemia, chronic renal failure, chronic pancreatitis, chronic diarrhea, gerd, diabetes mellitus, copd. in other words, i am totally confuse of how to get started on this. i dont get my professors tutoring. can someone please help me?

what you are asking would take a textbook. nursing diagnoses are based upon the symptoms a patient has. so, the best way i can explain this to you is for you to look up the signs and symptoms of these medical diseases first. then, using a nursing diagnosis reference book, find nursing diagnoses that have some of those symptoms and match with your patient. every nursing diagnosis has signs and symptoms just as every medical diagnosis has signs and symptoms.

read these two threads on how to construct a care plan and choose nursing diagnoses. it may help you.

it is helpful to have some sort of nursing diagnosis reference. i use nanda-i nursing diagnoses: definitions & classification 2007-2008 published by nanda international. a lot of students in the united states like to use nursing diagnosis handbook: a guide to planning care, by betty j. ackley and gail b. ladwig because it also has nursing interventions in it.

As an example, hypokalemia has the following symptoms and lets assume your patient has these symptoms:

  • diarrhea
  • muscle weakness and fatigue
  • dizziness

With those symptoms, the following nursing diagnoses can be extrapolated:

  • Diarrhea related to electrolyte imbalance as evidenced by more than 3 loose stools a day
  • Fatigue related to electrolyte imbalance as evidenced by lack of energy, muscle weakness and complaints of being tired
  • Risk for Injury related to dizziness caused by electrolyte imbalances

Problem(NOT medical DX..nursing diagnosis.) RT (what process is causing this....failure of heart to pump, GI motility, Stomach upset) AMB (the S/S of the problem) RISK for dx's do NOT have a AMB as this is NOT a visible problem yet.

(also, Risk Dx's are NOT high-priority, in general.)
