first you have to decide where you want to work then go to http://www.ncsbn.org then find the website of the state where you intend to apply. then go to their site and download the application form. instructions are there.

welcome to the site, plenty of information especially in the stickies on this forum. You need to decide where you want to live and apply to that site although if wanting to go to CA would suggest another route and then endorse. Plenty of info on the reasons why not to go CA route also on here. Also if you go to the home page and scroll down you can also find the US nursing BON 's which has links to their websites. You then just follow instruction for foreign nurse graduate

You also will be applying for licensure as an RN in the US to a particular state, you do not apply for the NCLEX exam. You are required to have permission of a state to be able to sit for the exam.
