where to get live scan form

Hi I am trying to get cna certification by equivalency. I had no problem finding the application, but where do I get the live scan form? All I can see from the website is a sample. thanks.

When I lived in California, I was able to get fingerprinted on a Live Scan machine at my local courthouse back in 2001. When I completed the LVN program in 2005, a person from the state came with a mobile Live Scan machine and fingerprinted all of the graduates.

i just went to the ups store and it was like 15 dollars i think, and im sure u have a ups store in your area :D
