I had a question..During rinseback of a dialysis patient. If you have forgot to give their Iron or any other med. DO you mark it as forgot and give next treatment. Isn't it dangerous to just give it to them during rinse back in fear of pushing an air bubble through? Has anyone experienced or seen this?

Well IV iron generally has to be given over 2-5 minutes so I would not give it during rinse back; it would be permissible to give epogen or zemplar. If given per procedure you wouldn't have any air present in your syringe.

Yes if given during the dialysis treatment gradually I know is ok. But what if the nurse forgot to give it through the machine to the patient and at rinse back just decided to push it all through? then it caused and air in blood and caused the patient to have burning of the lungs and lungs closing up and choking could not breath. had to be put on oxygen.. is it dangerous to just give it at rinse back and not during treatment?

My concern would be that the IV Fe was given at too fast a rate.

How are you more likely to give the patient air with a med during rinse back than at any other time? My concern about giving a med during rinse back is that the patient wouldn't receive the full dose. Some of it will be left in the saline unless you give it at the very beginning of rinse back.

So tonight I was doing a footcheck on a patient for the first time me doing it. He comes by stretcher for tx. he's 350 pound diabetic, who needs bilateral hip replacements, Lives at a nursing home. ANYWAY, his toenails were about an inch long on all his toes. I have not seen his shoes off before as another nurse was checking him. ( that's another story) What can I do about this? He says they haven't cut them since February, which, Um, is coming up. It's disgusting. He's diabetic!! How is this not neglect? Is there anything I can do to scare them into taking action? The nurse at the nursing home said "He's on the list" Not too encouraging. Thanks for any advice
