Filling Out VISA SCREEN APPLICATION FORM

hi!!

i need your help,especially those who had already been into this process. i'm confused with some of the questions found on this form.

for question under registration/license:

c. in which other country or countries do you currently have, or have ever held a nursing license?____

registration number_______

d. if licensed in the united states, canada,india or australia, please list the state or province in which you were licensed?

( i have taken my nclex-rn last year under california state, and since i still cannot provide them with the ssn,i do not have my license card, is it safe to fill in the blank with none as an answer??

q16. if you passed either sbtpe or nclex-rn, please list date and location where you passed the examination? ( i wrote down the date and place of my examination)

did passing of this exam lead to license being issued in the same state/province or country?yes__no__

( which one should i mark??)

i'm sorry ,so confused with these questions,and i dont want to proceed with the mailing of my application without being clear on these thing. i hope you could help me sort out this one.

do you hold a local license, if you do write down the information. you do not have a us license. you have passed the nclex.

I think it's a pretty simple form to follow.

C. Which country do you have a valid RN license from? Or if you've ever held a RN license. What is the license number?

(If you don't have a license you have to say NO, if you have a license, list the countries where you have a valid license, and give them the number)

D. If you have a license in any of these countries.

(If you don't have a valid license on hand, then the answer is no)

Q16. If you've passed the NCLEX and at which location.

(you already filled it out. If you don't have a valid SS# and you took your exam in a state that requires you to

have a valid SS# before they issue you a license, then the answer is NO you don't have a license. Passing the exam doesn't automatically give you a license. The key words here are "was there a license issued". If you didn't receive a license and your name isn't on the BRN website then the answer is still NO)
