fall charting

It will depend entirely on the charting system and policies and procedures for your facility. Usually there is documentation of what happened, vitals taken, physical assessment done and who was notified and when

Your supervisor will provide an example of the charting expected in your facility.

This.

We first assess and get vitals before getting the resident up. Do vitals again.

We have about 4 papers we chart on as well as calling DON, Physician and family. If the fall was unwitnessed or hit their head, we do neuro checks. We also need to figure out a fall prevention.

Pretty much find out from you facility and ask for a copy of the forms that need to be filled out so you have an idea ahead of time. There should also be information in your procedure manual too.
