Give me an example what you "bomb at".........many times the problem is not thinking globally enough/laterally enough to encompass the entire patient.

Are you using resources or just guessing? I've found my textbooks and google work wonders :) Without using anything, I do terribly. I don't know why, but I usually bomb them.

I use any resource I can, text, pdr, google. It's sometimes the way the choices are listed. In this example for the multiple choice answers it was the same question rephrased three different ways all would elicit the same answer from the patient. So how do you choose which one? I don't know if I can post the exact question as they are not due for another week. Does this make sense? I apologize if it doesn't I'm also on little sleep.

I'm not asking for the question....I have Ackley: Nursing Diagnosis Handbook, 9th Edition and Gulanick: Nursing Care Plans, 7th Edition are these case studies like this? or just questions.

Blah, Blah, Blah......is a 28-year-old mother of a 6-week-old baby boy. She is college-educated and will be returning to work soon. She has always been very "health conscious." She has interviewed three pediatricians and has chosen one who seems to share her views on child rearing. She intends to be involved in her child's health care and desires as much information as possible to make appropriate decisions. She is concerned because of stories she has read about problems associated with immunization.
Nursing Assessment
Mrs. D, 28-year-old college-educated woman. Infant son 6 weeks old making first well-baby visit. The baby did not receive some of the initial immunizations at birth. The parents decided to "wait." He had an Apgar of 10. He is being breast-fed. His weight and length are now within normal limits on the growth chart.
Mrs. D has many questions about immunizations. She wants her baby to be healthy, but she is unsure about potential side effects. She has brought along an alternative schedule for her baby's immunizations.

You need to look at the whole scope.....all that lateral thinking. Include disease process. Include normal process. Include potential disease process.

Key things I see from above.....a difficult example. This patient is wanting to be a good parent but she is also anxious...she interviewed 3 pediatricians to find ones that shares HER view. Well baby visit...the baby "is now" within normal limits....baby is breast fed....did baby/Mom have issues with breast feeding and nutrition/failure to thrive at first? Mom shows readiness for learning and immuniizations but has brought her "list".

Go to your book associated with nursing diagnosis......I use Ackley here and look up everything that fits.

Anxiety, Effective Breastfeeding, Ineffective Breathing pattern, Caregiver role strain, Readiness for enhanced Childbearing process, Readiness for enhanced Immunization status, Risk for Infection...do you see where this goes?

Look up on the internet. For disease processes/treatment/symptoms etc..... I use medscape/medline/emergency medscape/medscape nurses.

Are you talking about the ones that are testing your communication skills? Like when it asks you how you would phrase something?

I've done ok with the disease processes but sometimes its the communication questions and how should the nurse respond. I analyze each one. For example is it a closed ended question, does it follow the question asked, is it a blocking question and of course it always comes down to two choices and for some reason I get the wrong one.
