Entry required?

Hi, 

we have a patient who admitted and left AMA within 4  hours.  The only thing she would let us do is take her vital signs, is an entry and then a discharge required?  She has Blue Cross Medicare HMO.

I'm lucky I even know how to spell out MDS! I did it soooooo many years ago. It's all pretty much a foreign language to me now. That's why I said I was guessing.
