Today I was reading about a care plan report card that you can send to the residents family member who does not come to the care conference meeting. I really like this idea but have no clue what a template would even look like. It would be really nice to ensure we "communicate" with family, especially those families that we have never seen! Sometimes I feel like the only time we talk to some family is to "notify" and that is usually not a happy call. Any ideas? Thank you all for being here:cool:

The report is probably based upon all the disciplines. We have cue cards that we carry that include how much assistance is needed for ADLs, transfers, ambulation, eating, AM/PM cares..their diet, if their DM etc. We input the information and our computer compiles and prints it for us. The thing is, it has to be updated which I catch wrong things and have to get them fixed. It also pulls info from the care plan. ALL CNAs have to carry one. I stick one in my pocket but only have to use it for new people. I am there almost every day and know what's going on. (I still scan it though looking for errors)
