Could someone decipher this for me please?

Help! I have a patient in the a.m. (this is our first time reviewing the chart the night before) and I would like to have an understanding of what this means.

Diet: 1/15 mid gm Na+ (I understand the sodium/gram part here LOL that's it!)

Act: 1/15 up ad lib (what is the 1/15?? again...)

Lab: 1/15 SCP q6 X 2 (there's that darn 1/15 again...SCP?)

xray: 1/15 R shoulder xray 3 view (understand it all but the 1/15.....)

1/15 dipy thal (dipy? this is a cardiac pt on cardiac tele floor admitting dx is PSVT...I actually KNOW what that one is!!!)

Any help on that "1/15 bit" would be greaty appreciated!!!

Are you sure the 1/15 is not the date the order was written?

Yep,, i agree,, 1/15,, date order written by dr. and added to patient care list.

Now go look yourself in the eyes in the mirror, bop yourself on the forehead,, go duh,, and know there will be many more moments like that. (We all have em),, ;) Happy clinicals!

Dipy thal is a persantine stress test. Dipy is an abbreviation for the generic name of the drug.

But the PT wasn't admitted until yesterday afternoon...1/17....? She had only been there for a few hours when I got her chart....Hmmmm....

We have a clinic connected to our hospital and a lot of times they have outpatient procedures. Sometimes these orders show up within the chart and sometimes not. Maybe they had all this done on an outpatient basis? And then they were admitted on the 17th....:rolleyes:

Thanks everyone! I'm heading off to the hospital in about 30 mins...from what I gathered she was in the ER then admitted to the floor yesterday, so I am not sure if those were done on an outpatient basis. I suppose it is possible too that the doc had the wrong date, as previous poster said...thanks for helping me think it through!
