So I was looking at my copy of the COC form after I tested the other day, and noticed the collector wrote 2018 as my birth year by mistake...will Quest still run it? Has this happened to anyone else?

I haven't had this exact same thing happen, but I did have another error. Mine was they checked the box for pre-employment instead of "random". In my case I called Quest and they said it didn't matter, and then I called my case manager and he said as long as it wasn't a change in which test was ordered (like option 1 instead of option 3 or whatever) that it didn't matter. In your case, the age of the patient shouldn't really change the test results so it should be fine. You could call your case manager just to be sure, but I doubt there would be any issues with this. My guess is that whenever someone sits down to enter the info into their system, they will see the error in birth date and go back and find out what the correct year should've been.

I have not had this happen, but I did have a collector accidentally check the "no" box where is asks if the temperature was within range. I sweated it for a few days but it came back negative. Hopefully everything is ok in your case as well.
I also had where they said the COC number did not match the COC on the specimen cup. This made no sense to me as it was an online COC. In the end, the test was rejected,
Worst case is they reject the test and refund you I believe, and then of course add another test.

This is only one of my reasons why I refuse to go anywhere but to the same LabCorp location every. Single. Time. Primarily it's because I won't give this extortion racket a nickel more than I must in 'collection fees' but also because I don't have to deal with those paper forms and risking someone screwing up how the form is filled out. It's also usually one of the same couple of people there so the humiliation is kept to a minimum and I have some reasonable confidence that they know what they are doing.

I've had at LEAST 10 tests that came back FLAWED TEST which I believe means something was wrong in the COC and they have to call the testing site to verify something . ( I'm assuming that's what they each mean because I've emailed the lab rep a few times about it because the results were taking 2 plus weeks and I've had to email pictures of my copy of the COC several times to verify). Also my last 3 tests results say yes in the observed column, which they are not. So someone keeps screwing up like constantly. It causes me so much stress, anxiety, and worry it makes me sick, literally!

This is All hasn't made me a better nurse.. it's turned me into a very negative angry ball of stress!
What ended up happening WestTX?
They ran the test and never said anything about it. Thank goodness!

I just had a test that wasn't observed that they documented as observed. Has that every happened to anyone?
