You close the clamp to the bag and leave the air clamp open. You need to make sure the bag of fluid you are using is compatible with the drug. In this case, vanc and saline can go together. You would dilute it like you normally would - I would normally put vanc in 100ml of saline. So put in your drug and fill the burette up to 100ml. Program the pump to run a volume of 100ml over 1 hour (or however quickly you want to give it). When it's finished, I normally put a 10ml flush in the burette and run it through, then open it back up and continue to run the fluid like normal.

I think I am still confused. when the medication is run through how do I let the normal saline just flush through like normal through a burrette? Do I keep the air clamp open the whole time? What about the roller clamps? What amount of fluid should be put in and what should I program the pump as based on this book. "Dilute reconstituted solutions to 5mg/ml (500mg/100ml) or less with either glucose 5% or sodium chloride 0.9%. Administer over at least 60 minutes for 500 mg dose."

Once the vancomycin has finished you open the clamp to the normal saline to allow 20-30mls into the burette for the flush, then you clamp it shut and make sure the air valve clamp is open. Then just program the pump to administer the flush.
