Brain sheet for 12+pts.per sheet

I have rehab and LTC pts.Typically have 22-25 pts. I need a brain sheet that holds 10-12 pts. on a page.Anyone have one?

Using sheet that is set up with rooms and names in order, I leave a column for 8, 10 12 and 2 meds, a column for nutritional supps and mark if they are prn or scheduled. I then leave a wider area for what I need to do or pertinent notes; VS, CBGs, skin check etc and put it in boxes where ever I have to log something. When it is done I add the info to the box and then at shift end carry over what i need to the 24 hour sheet. I take the info from report and put it on the far right for reference. I review the whole MAR prior to starting as well as the TAR so I am not surprised later in the shift when tx needs to be done
