Q1. During the first stage of labor, do we assess contraction every 30 minutes; FHR every 60 min in latent phase, every 30 minutes in active phase, and every 15 minutes in transition phase; vital sign every 2-4 hours; and BP every hour?
Q2. how about the schedule of assessments during the second stage of labor? Do we still assess contraction every 30 minutes, FHR every 15 minutes, vital sign every 2-4 hours, and BP every hour?
Q3. how about the schedule of assessments during the fourth stage of labor?
Assume the client is low risk.

You should check your hospital poilcy. For the hospital where I work, Temp is taken every 4 hours until ROM then it is q2h. VS are taken every 15min x2 hours, then hourly on all laboring patients.
Fetal monitoring is continuous with FHT and contraction pattern recorded q30min during latent phase. If on Pit or 2nd stage of labor FHT are recorded q15min and contraction pattern q30min,

Hospital policy is important but ACOG and AWHONN clearly state the minimum accepted amount of assessments as well. I don't have time to look them up for you but everywhere I've worked has followed ACOG/AWHONN or is even more strict. Good luck researching!
