how to apply visa screen

sir i applied for the vsc and it in process now. i already mailed prc license and school validation to CGFNS

how do i comply with their remaining requirements:

1) vermont license validation,

2) passing the NCLEX, and

3) passing english test

vermont has several verification forms online, i am confused what form should i use. how is it going to be?

will bon directly send the verification to cgfns?

VSC did have their own verification form in the handbook. That form when sent to the BON will confirm NCLEX pass. English if not exempt must be sent directly from the establishment you took the exam

Sir is it the same VOL form prc had filled up?

I checked bon of vermont they have their own form of VOL stating that i authorize vermont secretary of the state to furnish verification to a licensing authority. the form also states that i should enclose a check payable to office of the secretary.

yes you need to pay any fees and the state form should be OK

Sir how about the english test results is it enough that i just supply them the report number of my ielts test result in their site or I should send them the original copy of test results.

Your English results must come direct from the company. You can not send in your results

sir i am really confused about the verification of license of vermont. they have their own form and it does not specifically indicate the form for visascreen. i am hesitant that i might fill up the form in the wrong way and i have to send and spend an expensive fedex again. I am dying to get guidance.

I suggest removing the thumbnail as I can still see bits of information. You can complete the form as it is going to CGFNS and then just let CGFNS that you have arranged for it to be sent to them and they should put it in your file

i already applied for ces at cgfns before and now i am going to apply for the vsc.

their instructions include printing of payment voucher, photo id form, etc.

i am confused if i still need to mail the payment voucher print out even if i had already paid the fee thru online with credit card? i am afraid i will spend an expensive philippine courier and they will charge me again.

You only need to send the paper payment form if youre not able to do it online.
