Care Maps- Anyone done them?

If anyone has done the care map form for care plans, I would appreciate any suggestions that worked for you.

when i was student nursing we had care maps along with our long list of other paperwork to do. But anyhow this is what i can remember.

each thing was in a box and it went down the page like so;

1. nursing diagnosis

2. goals

3. any labs, meds, treatment being done

4. interventions by nurse, rationales

5. describe how and when the goals were met, specific time frame

If anyone has done the care map form for care plans, I would appreciate any suggestions that worked for you.

I am a 2nd semester ADN student, and we are doing concept maps with separate Impression (a global evaluation). It was all new to me this semester as last semester all we did was the clinical logs with nursing diagnosis, goals and interventions.

I have completed 2 concept maps so far...working on my 3rd this week (after the exam on wednesday). ...and I really like them!

...it is quiet lengthy, but helpful in getting started on ideas.

I also work my concept map in Word (Microsoft 2000). (I prefer to type than handwrite anyday!) :chuckle

My instructor likes my concept maps so much, that yesterday she told the whole clinical group that if anyone needs help on it, to ask me! (didnt expect that!!!) :rolleyes:

basically I set it up like this:

Start w/a box in the middle of page, with the primary dx and other dx listed

then use outside boxes to list:

nursing dx (numbered in order of priority)

r/t ...

aeb...would include any labs also

interventions...would include any medications (including dosage, route, freq)

then I draw lines/arrows from the primary dx box to each nursing dx box, then lines from nursing dx box to other nursing dx box as they relate to each other.

Then work the impressions in order of priority, numbering them to correlate with the map.

We purchased the book Concept Mapping: a Critical thinking Approach to Care Planning the ISBN# is 0-8036-0979-5. The book takes a paiient and runs through the entire concept mapping process. It might help. i would also buy a care plan book if you don't already have one to give you nursing diagnosis and interventions. I am a senior, in my last semester and we are thank God done with care plans :balloons: :balloons: They suck

Amen! We had to buy this book for our summer semester (3rd one). Of course, we had a fit. "Why didn't we know about this book when we started doing the stupid maps in the first semester!!!" This book is great for starting out, and has great tips for clinical, period. -Andrea
