As I posted before I was transferred to a new office the pedi there is not my supervising md. Well today was my first day at the office. All was well until this afternoon and one of his Pts was seeing me. This was a preemie now 2 months old has only gained 12 oz since birth wt today was 4 lbs 15 oz and hypothermic did not look well so I asked him if he could take a quick perk at her his comment was I am not your supervising md call her. I was like huh this is one of your Pts I wound up. Sending her for a septic work up. Needless to say I thought this was unprofessional. He has a rep for being like that just some feedback from you all if should have handled it different

Whew - that's not really what you needed. I would personally be concerned about the level of assistance you have in that practice (from all the problems you've posted about).

I guess in this situation, like you, I would have been very conservative and yep, send the kid to the ER for a septic w/u or admit them under your supervising MD's name for a septic w/u.

i was not able to get ahold of my collabarting md, I did speak to administration and they were not pleased with his response basically they felt that even though he is not my supervising md it is his patient and he should have at least taken a quick look at her, he is worried about liability if he should give me any type of input. My collabarting is not even on sight and has no problem with advising me after I have told her what my observations are. So I am just going to keep on doing my job and stay out of his way. Thank goodness we are at other ends of the building

Wow! Very unprofessional, passive aggressive and, quite frankly, legally risky on his part. Sounds like you handled it very well. Make sure you document!!

Well needless to say administration is not happy with him, and now to top it off, he told administration that he told me to call 911 which he did not, so he is lying to cover his butt.

I think the writing is on the wall from all you have written. If you have no support, there are bound to be mistakes made just because you are still so new. I doubt these docs are going to support you or back you up.

I would never want to work anywhere there is physician hostility and you seem to be right in the middle. I think for the sake of your license you need to call an emergency meeting somehow of all of those involved in the decision for you to be away from your collaborating physician. Additionally, the fact that the MD currently on site will LIE to protect himself is another reason to not practice until this gets fixed.

The CEO and Vice CEO are having a meeting with him on Monday. He has a reputation for being tempermental, and trying to push people around. In the past his nurses only lasted 8 months on avg because of him. I dont know why the last provider left. I think I am the first mid level he has ever worked with and is not sure what to make of me. I spoke with my supervising MD and CEO yesterday and let them know I will not stand for his underhanded tactics and unethical practices. As the CEO stated what he did was ethically incomprehensible and my collabartive MD stated that what he did was at the least ignorant and uncalled for. Admin and my collabartive md are very supportive and angry that this happened to me. And now that they know he lied to protect himself has gotten him into hot water. I had 2 witnesses standing there when he told me that he was not my supervising MD and I needed to call her. These 2 witnesses have already spoken with the CEO and my MD and told them the same thing I had told them. It is really sad because I love my pt population and the staff and admin is backing me up but if they cant do something to fix this then I am asking or a transfer. I am not going to risk my reputation or license. Thanks guys you are better than anxiety medicine anyday

I would be concerned about working in a place where I get no support. I would not do it. I am a NP. I work hard to do a good job and treat pts as they need to be treated, but I do need physician backup on some of my patients-for crying out loud the ER docs I work with ask each other advice from time to time or call specialists for advice if needed.

Sounds like a very hostile environment. I'd ask for a trandsfer too.
