What EXACT documents?

Ok, so with an old misdemeanor conviction I am going to need court documents.

Which documents do I EXACTLY need? I'm not good with all of these court papers!

Do I need the paper....

1. Says my formal charge

2. Says my agreement, like defendant will do 20 hours of community service...and so on

I just want to make sure I have it all there to not delay anymore than it already will be.

Contact the BON to be absolutely sure you have the correct information. Different states may have different requirements.

If you haven't done so, you should visit the website for the state board of nursing to which you are applying. You should be able to find this information there. If not, and you have to contact the BON you might consider doing so via email; most state BONs have a generic email address and they will be able to forward yours to whomever can best answer the question.

Most likely, you are going to be asked to provide a detailed statement regarding your conviction. I am unsure which state you will be applying to, but as an example Pennsylvania requires the following: A detailed, signed and dated personal statement explaining the action, its background and any rehabilitation.” If the faculty at your school are aware of your situation, one of them might help you draft this letter.

You are also likely to be required to provide copies of the criminal court documentation detailing the charges, conviction, and sentencing. The clerk of the court in which your case was heard should be able to provide this information.
